Through our continued successful delivery of our services, we continue to deliver the National Diabetes Prevention Programme across the UK. A programme that has helped thousands of people across the country.
NHS England, Public Health England and Diabetes UK have committed to improving efforts to prevent diabetes and this collaborative programme aims to help the 5 million people who are at risk of developing the disease.
What you will be doing
As the Engagement Officer, you will liaise between the NDPP service, primary care, and other public health stakeholders to increase awareness of the programme and to improve referrals into the service. This will also include supporting the engagement into the Diabetes My Way programme.
You will be offering direct support to GP practices to ensure accurate monitoring and coding of patient outcomes which will support onward reporting to NHS England.
Working alongside your Area Service Coordinator and Regional Manager you be developing key working relationships that will have a positive impact and engagement towards the NDPP.
Who are we looking for?
First and foremost, we are after someone who has a background in primary care, NHS or Public Health. We are also after someone who has excellent relationship management skills.
If you have background in health and wellbeing that will be a huge bonus for us.
