Provide support, advice and guidance to Children Looked After and Care Leavers aged 16- 21 years old. Offering support in moving accommodation, accessing EET opportunities, building on independence skills such as budgeting, supporting in accessing other services such as physical health, mental health or youth groups that can help reduce isolation and loneliness. As a Pathway Advisor you will be expected to write SMART pathway plans with your Young People, setting them realistic goals and review these every 6 months and you will also be required to complete In Touch forms every 8 weeks. Young People live all over the country which will mean travelling, a driving license is desired. As working part of the team there are also duty days you will be required to work in the office. Duty shifts are either 8am-4pm or 4pm-8pm. A rota will be shared with you.

You must have:
A professional qualification in social work and experience of working with Adults, in particular previous experience of Assessment work
Social Work England registration or Social Care Wales registration for North Wales.
Post qualified experience.
Enhanced DBS/Update service.
References required for the last 5 years of employment
