Planned Parenthood of Greater New York is seeking aPatient Referral Coordinatorto join our Clinical Services team for a full-time 37.5 hour work week. This position will be based at our Kingston Health Center and will travel regionally, including to our Poughkeepsie and Newburgh centers.

What a Patient Referral Coordinator does:

The Patient Referral Coordinator is responsible for coordinating, processing and scheduling of medical referrals for patients in need of external consultation as requested by PPGNY medical providers. This position is also responsible for tracking, and providing reports. The Referral Coordinator collaborates with social workers, nurses, and other patient care support staff as needed.
Monitors electronic health record (EHR) notifications throughout the day to capture referral request in real time.
Gathers pertinent information as needed via EHR or from clinical team to accurately complete pre-authorization/referral process.
Schedules referral appointments directly on behalf of patient.
Utilizes third party payer/insurance portals; has an understanding of insurance providers, their portals and their expectations for authorization approvals.
Maintains an accurate records of external referral patient's appointments that are kept, failed and current status utilizing the EHR or other established system.
Responsible for running referral reports as requested.
Routinely contacts patients to remind them of their referral appointment by phone, mail, text, patient portal or other mutually agreeable method.
Responds to in-house provider questions, requests and concerns regarding the status of patient referrals or follow-up status.
Answer, respond and document in the EHR phone calls, requests and questions from patients in a timely manner. Calls must be accurately managed or redirected as appropriate.
Assists patients in accessing other services needed to maintain their referral appointment such as but not limited to transportation, interpretation services, health care benefits.
Ensures that all referral forms and returning consultant reports are located in the patient's electronic health record in a timely manner.
Minimum Qualifications

High School Diploma /GED
4 year work experience scheduling calendars, general correspondence, customer service
Work experience with computer systems utilizing web browsers, search engines, data entry and MS Word
Accurate keyboard skills and attention to detail
You are interested in expanding your analysis and knowledge about the role that racial inequity plays in our society, and are committed to building and/or deepening your commitment to racial equity in your work
Preferred Qualifications

Bachelor’s Degree
Work experience in healthcare industry as medical receptionist, patient coordinator or patient/member services
Work experience utilizing electronic medical records systems
